Nurse Practitioner ( NP ) for our Long Term Care Facility ( LTC )

Compensation: $135,000 - $145,000 Annual Salary

Benefits: Medical, dental, vision, life insurance, long-term and short-term

Position Description:

We are looking for a dedicated and compassionate Nurse Practitioner ( NP ) for our Long Term Care Facility ( LTC ) for bedside patient care. This is a full time position, Monday - Friday 9am - 5pm.

Qualifications:

Valid Nurse Practitioner ( NP ) license in the state of Missouri
1+ years of Long Term Care experience

Responsibilities:

Upon admission to the facility, examine residents and their medical records.
Check ups, management of chronic conditions and medications, transitional care management, discharge planning and family engagement.
