National Care Consortium

The National Rural Accountable Care Consortium recently received an award for up to $31 million from the Centers for Medicare and Medicaid Services' (CMS) Transforming Clinical Practices Initiative (TCPI). We are currently in the process of developing Practice Transformation Networks (PTNs) that prepare providers for the movement to value-based payment models. This program is offered to you at no cost!

TIMELINES:
Training and support is ongoing through webinars and workshops. These will be available through our online learning management system. We hold quarterly regional Quality Improvement Workshops in April, July and October for face to face meetings with the teams. I have included an electronic form of our brochure and an FAQ.

ELIGIBILITY:
Practices can "graduate" from our program and move into an Alternative Payment Model of their choice - ACO, MA plan, new models. Eligible practices include RHCs and any practice that is eligible for PQRS. NOTE: Practices cannot already be part of any Medicare Shared Savings Program (MSSP, Pioneer, NextGen, CPCI, etc.) and cannot be currently participating in any other CMMI Practice Transformation Network. Bundled Payment participants are not excluded from participating.

The participation agreement is a great place to understand expectations of both you and NRACC. This program is offered at NO COST TO YOU and you can exit the program anytime. The application and contract can be completed and returned to us online. I have attached our TCPI Fact Sheet and brochure for your review.

The major program elements are:
  • Setting up your on-site billable care coordination program (includes IT/data analytics infrastructure and 24-hour nurse advice line)
  • Redesigning practice workflows to manage population health
  • Preparing you to become a Patient Centered Medical Home (PCMH)
  • Increase your revenue through improved primary care and billing
